1. Acceptance of Terms

By accessing or using our services, you agree to be bound by these Terms and our Privacy Policy. If you disagree with any part of these Terms, you may not access our services.

2. Services Description

2.1 Concierge Nursing Services

AmeriAxis Health provides licensed practical nursing services including but not limited to:

• Medication management and administration
• Wound care and dressing changes
• IV therapy and infusions
• Post-operative care
• Health assessments and monitoring
• Chronic disease management support

2.2 Wellness Services

• Health coaching and education
• Lifestyle modification guidance
• Preventive health strategies
• Corporate wellness programs

2.3 Service Limitations

Our services do not include:

• Emergency medical care
• Primary medical diagnosis
• Prescription of medications
• Replacement for hospital care

3. Scope of Practice

3.1 LPN Practice Boundaries

All nursing services are provided by Licensed Practical Nurses practicing within the scope defined by the Arizona State Board of Nursing. Services requiring RN assessment or physician diagnosis are coordinated through our network of qualified providers.

3.2 Collaborative Practice

We maintain collaborative agreements with Registered Nurses and physicians for services beyond LPN scope, ensuring comprehensive care coordination.

3.3 Client Responsibilities

• Provide accurate health information
• Follow agreed-upon care plans
• Notify us of changes in health status
• Keep scheduled appointments
• Provide safe environment for care delivery

4. Payments and Fees

4.1 Service Fees

• Hourly rates for nursing services
• Monthly retainers for concierge clients
• Package pricing for specific service bundles
• Corporate program pricing

4.2 Payment Terms

• Payment due at time of service unless otherwise arranged
• Accepted payment methods: credit cards, debit cards, HSA/FSA
• Corporate accounts: net 30 days

4.3 Insurance

We are a private pay service. While we don't bill insurance directly, we can provide superbills for possible out-of-network reimbursement.

5. Cancellation and Rescheduling Policy

5.1 Appointment Cancellations

• 24-hour notice required for cancellations
• Late cancellations (less than 24 hours): 50% service fee
• No-shows: 100% service fee

5.2 Service Termination

Either party may terminate services with 30 days written notice. Emergency termination may occur for safety concerns or contract violations.

6. Liability and Disclaimer

6.1 Medical Disclaimer

Our services complement but do not replace primary medical care. Clients should maintain relationships with their primary care physicians and specialists.

6.2 Limitation of Liability

AmeriAxis Health's liability is limited to the extent permitted by Arizona law. We carry professional liability insurance as required by state regulations.

6.3 Indemnification

You agree to indemnify and hold harmless AmeriAxis Health from any claims arising from your failure to provide accurate information or follow care instructions.

7. Confidentiality

We maintain strict confidentiality of all client information in accordance with HIPAA regulations and our Privacy Policy.

8. Modifications to Terms

We reserve the right to modify these Terms at any time. Continued use of services after changes constitutes acceptance of modified Terms.

9. Governing Law

These Terms are governed by the laws of the State of Arizona. Any disputes shall be resolved in Maricopa County courts.
